Contract for Facilities Usage
The Jackson Medical Mall Foundation would like to thank you and your organization for considering the Jackson Medical Mall as a host site for your event. The Community Meeting Room, Center Stage and Mall Common Area are rented to accommodate Medical Mall tenants and neighborhood, business and civic organizations in their outreach efforts in communities around the metro-Jackson area. All of the above are reserved on a "first-come, first serve" basis and should be requested at least fourteen (14) days in advance.

1. The Center Stage, the Mall Common Area and the Community Meeting Room are not used for family reunions, wedding receptions, class reunions, baby or bridal showers, anniversary or birthday parties, religious or political events, concerts, dances, other social gatherings or "for-profit" fund-raisers that are not directly related to the goals and objectives of the Foundation. | |

2. The normal fees for usage are: | ||

· Center Stage/Common Area- $250.00 fee for up to four (4) hours and a $350 refundable deposit. An additional $100.00 will be added if event is over four (4) hours. | |

· Mall Common Area or | |
|
|
· Additional fees may be incurred if events require security, custodial, and additional maintenance for breakdown or setup at a rate of $15.00/hour. | |

3. A security deposit is mandatory for all programs. Deposits may be returned, given that the areas have no tape placed on the walls, and are left free of debris, in acceptable condition, and the equipment is not damaged or misplaced. The usage fee and deposit require two separate checks. | ||
I have read this agreement and I understand the policies of using the Center Stage, Mall Common Area and Community Meeting Room. Failure to abide by this contract will result in forfeiture of entire deposit and possible additional fees may occur. I am clicking accept to verify my understanding of these rules and regulations.
